Mounjaro & Zepbound Injection Sites: A Rotation Guide

The three approved injection sites, why rotation matters, and a simple system for tracking where you injected last.

Tirzepatide (Mounjaro and Zepbound) is administered as a weekly subcutaneous injection — meaning it goes into the fatty tissue just under the skin, not into a muscle. Where you inject matters more than most people realize, and rotating sites properly can significantly reduce discomfort, bruising, and the skin changes that come with weekly injections over months or years.

The three approved injection sites

Abdomen

The abdomen is the most commonly used site and generally the easiest to self-administer. Inject into the fatty tissue at least 2 inches away from the navel — avoid the area directly around the belly button, where the tissue is thinner and more vascular. The abdomen has excellent absorption and is the default site recommended in most clinical guidelines.

Thigh

The front or outer thigh — specifically the upper and middle portion of the thigh — is the second most common site. Avoid the inner thigh, which has less fatty tissue and more blood vessels. The thigh is particularly practical when traveling or in situations where abdominal access is inconvenient.

Upper arm

The fatty tissue on the back of the upper arm (the triceps area) is the third approved site. Self-injecting here can be awkward — it's the hardest to see and reach without assistance. If you use this site regularly, a mirror or a helper can make it easier to ensure proper placement.

Why rotation matters

Injecting into the same spot repeatedly causes lipohypertrophy — a buildup of fatty, fibrous tissue under the skin. You've probably felt this if you've ever pressed on the skin near a frequently injected area and noticed a firm lump. Lipohypertrophy isn't just a cosmetic issue: it significantly impairs medication absorption. The same dose injected into a lipohypertrophic area may be 20–30% less effective than one injected into healthy tissue.

Consistent rotation prevents this and ensures you're getting the full benefit of each dose.

A simple rotation system

There's no single required rotation pattern, but a systematic approach beats randomness. One approach that works well for weekly injections:

  • Week 1: Abdomen, left side
  • Week 2: Abdomen, right side
  • Week 3: Left thigh
  • Week 4: Right thigh
  • Week 5: Back to abdomen, left side (cycle repeats)

If you also use the upper arms, add them into the rotation as weeks 5 and 6, extending the cycle to 6 weeks before returning to any given area.

Within-site rotation

Even within a chosen site (say, the left abdomen), you should vary the exact spot each time. Think of each site as having multiple sub-zones — imagine a grid of points, each at least an inch apart, and work through them systematically. This gives each micro-location several weeks to recover before being used again.

What to avoid

  • Scarred or hardened tissue — avoid areas with visible scarring, lumps, or bruising from previous injections
  • Skin folds with stretch marks — absorption can be unpredictable
  • The waistband area — clothing friction can cause irritation
  • Within 2 inches of the navel — thinner tissue, higher vascularity
  • Anywhere you feel a lump — this is lipohypertrophy; let it recover for several weeks

Tracking injection sites

Logging your injection site each week alongside your dose and date serves two purposes: it helps you maintain your rotation systematically, and it gives you a record if you ever notice an absorption issue or skin change to discuss with your provider. Many people find that once they start tracking sites, they realize they've been defaulting to the same spot for months — which explains why they developed a lump they hadn't noticed.

A simple log entry — date, dose, and site (e.g. "Abdomen L", "Thigh R", "Upper Arm L") — takes seconds and is one of the most practical habits you can build around your weekly injection.
